Lutheran Home Care & Hospice Inc.

Lutheran Social Services of South Central Pennsylvania and Diakon Lutheran Social Ministries have integrated their home-care operations.

The integration of three Diakon Home Care programs — Diakon Hospice Saint John, Diakon Home Health and Diakon Help at Home — with Lutheran Home Care & Hospice was effective July 1. Review of final licensure is pending but expected to receive approval.

Staff members of the three Diakon programs have joined Lutheran Home Care & Hospice Inc. to provide home health, in-home support, home-health telemonitoring and hospice care to individuals and families from northern Maryland through northeastern Pennsylvania. All three Diakon Home Care offices in Allentown, Wyomissing, and Hazleton, Pa., remain in operation.

Lutheran Home Care & Hospice Inc. will continue to provide services in the programs’ existing service area, including Adams, Franklin, Fulton, Cumberland and York counties in Pennsylvania, and Carroll, Frederick and Washington counties in Maryland. With this integration, the organization plans to expand services to portions of central, eastern and northeastern Pennsylvania, with a concentration of services in Luzerne and Berks counties and the Lehigh Valley.
