And you are right, Obama did get the consent of the governed. Unfortuently for him, the majority of the governed are getting really upset by his governing.

Just your RESPONSIBILITY, not your ability or the current balance. That is similar to whether you are responsible for your children's finances for insurance purposes (it's a YES or NO thing). NOT what the balance is in your account at that moment. Lines 21-24 enables electronic funds transfer (read ATM card) to pay bills. It allows you to set it up, not forcing you to use it. It's for your convenience just like at Target or Sears or anywhere else.

Now when you compare some of the items in the summary to what is in the bill, it doesn't really sound like they are talking about the same things, but some of the more disturbing items are pretty much in plain english on the bill.

The underlying fact is that the heath care and insurance cartels have had three decades to reform health care and provide a reasonable service for a reasonable cost, they failed due to greed and incompetence now there is a need for a lower overhead system (administrators not making $500 per hour) and bilking the users for the exhorbinant salaries. The corporate healthcare warlords have proven without a doubt they will never be interested providing value, Now they will have competition. The healthcare charges have risen astronomically and not within any reason. It is gutting the US GDP. Perhaps the low overhead, high subscribership competition will force them to provide value or die off. The fear mongers like you just want us all not to have a choice and to have to go to the current cartels and be ripped off.
