Steve Nutt, the driver of the third car, said he saw the Ford LTD swerve into the path of the Honda when he reached the crest of the hill.
Nutt, of Inwood, said the man's car appeared "out of nowhere" when it hit the Honda.
After the cars collided, Nutt said he went to the Ford LTD to check on the man.
"He was moving a little bit. I was just telling him to hang on. I told him to hang on, we got an ambulance coming," said Nutt.
Nutt said he then went to the Honda, and the woman was frightened because it was starting to smoke.
Police did not have the names of the woman and two children traveling in the Honda. Their conditions were unavailable this morning.
It was the second fatal car accident in Berkeley County within 24 hours.
Perry Prather, 38, of Falling Waters, was killed Monday night when his 1966 Ford pickup truck went off U.S. 11 north of Martinsburg near Downy Orchard Road and struck a tree, said Lt. Dennis Streets.
Woman, 70,struck, killed
EMMITSBURG, Md. - A 70-year-old woman was struck and killed Tuesday afternoon as she crossed Old Frederick Road in front of her home to get her mail, Frederick County Sheriff's deputies said.
Irene Agnes Wantz, of 15809 Old Frederick Road, was at the center line of the road at 2:25 p.m. when she was hit by a southbound 1995 Pontiac Grand Am driven by 19-year-old Kyle David Lewis of Thurmont, Md., deputies said.
Wantz was pronounced dead at the scene, deputies said.
Deputy 1st Class William Folden is continuing his investigation into the accident.
A split rail fence valued at $500 was taken from a home in the 1100 block of Eastwood Drive, according to the Washington County Sheriff's Department.
Police said the theft was reported Tuesday.
Md. tag stolenfrom motor home
A Maryland tag was taken from a 1968 Ford motor home parked at a residence in the 13700 block of Lois Street, a Washington County Sheriff's deputy said.
The theft of the tag numbered, 115484 M, was reported on Monday.
